Welcome to Next Level Physical Therapy and Sports Performance LLC (“Company”, “we”, “our”, “us”). By accessing or using this website or purchasing any services or products, you agree to the following Terms and Conditions and our Privacy Policy, which is incorporated herein by reference.

Scope of Services

Physical Therapy Services: Physical Therapy Services are provided in-person only and are not available for purchase through our website. All physical therapy services are governed by separate intake forms, consent documents, and financial policies provided in-office. Nothing on this website constitutes physical therapy evaluation, diagnosis, or treatment.

Coaching Services: We offer fitness and performance coaching services delivered remotely via a third-party platform. These services are not medical care, not physical therapy and are not intended to diagnose, treat, or manage any medical condition. By purchasing coaching services, you acknowledge that you are participating in a general fitness program at your own risk.

Digital Programs: We may offer pre-written, sport-specific or general fitness programs for purchase. These programs are not individualized, do not include coaching or communication, and are intended for independent use. These programs are not physical therapy and do not establish a provider-patient relationship.

No Medical Advice

All content provided on this website, through our coaching services, or within digital programs is for educational and informational purposes only and does not constitute medical advice. Participation in any exercise program carries inherent risk. You should consult a qualified healthcare provider before beginning any program.

No Provider-Patient Relationship

Use of this website, purchase of digital programs, or participation in coaching services does not establish a physical therapist-patient relationship. A provider-patient relationship is only established through an in-person evaluation and completion of required documentation.

Assumption of Risk

By purchasing or participating in coaching services or digital programs, you acknowledge and accept:

  • Risk of injury or adverse outcomes

  • Responsibility for your own health and safety

  • That you may stop any activity at any time

You voluntarily assume all risks associated with participation.

Payment, Billing, and Subscriptions

Coaching Subscriptions

Coaching services may be offered on a recurring subscription basis. By enrolling, you agree that:

  • Your payment method will be charged on a recurring basis (e.g., monthly)

  • Subscriptions automatically renew until cancelled

  • You are responsible for cancelling prior to your next billing cycle

Cancellation Policy and Refunds

You may cancel at any time by emailing chrismart002@nlphysicaltherapy.com or messaging through the CoachRx app. To avoid being charged for the next billing cycle, cancellation must be submitted at least five (5) business days before your next billing date. All sales are final. No refunds will be issued unless required by law.

Third-Party Platforms

Coaching services may be delivered through third-party platforms (e.g., CoachRx). We are not responsible for platform outages or technical issues, data loss, or service interruptions caused by third parties.

Intellectual Property

All content, programs, materials, branding, and information provided by the Company are the exclusive property of the Company. You may not reproduce, distribute, or share programs, resell or republish content, or use materials for commercial purposes.

Website Use

This website is intended for informational purposes only. While we strive for accuracy, we do not guarantee that all content is complete, current, or error-free.

Prohibited Use and User Conduct

By using this website or purchasing our services or products, you agree not to: 

  • Use the website or services for any unlawful purpose

  • Misrepresent your identity or provide false information

  • Attempt to gain unauthorized access to any portion of the website, systems, or client accounts

  • Interfere with or disrupt the functionality or security of the website or third-party platforms

  • Copy, distribute, share, or resell any programs, content, or materials without prior written consent

  • Engage in abusive, threatening, or inappropriate behavior toward the company or its representatives

We reserve the right to suspend or terminate access to services or content without refund, if we determine that a user has violated these Terms. We further reserve the right to pursue any legal remedies available for violations of these Terms.

Limitation of Liability

To the fullest extent permitted by law, the Company shall not be liable for any injuries or damages resulting from use of this website, participation in coaching services or digital programs, reliance on any information provided.

Governing Law

These Terms and Conditions are governed by the laws of the State of South Carolina. Any disputes, claims, or legal proceedings arising out of or relating to these Terms, the use of this website, or purchase of services or products shall be exclusively brought in the state or federal courts located in Beaufort County, South Carolina, and you consent to the personal jurisdiction and venue of such courts.

Governing Policy Relationship

These Terms and Conditions should be read together with our Privacy Policy, which explains how we collect, use, and protect your data. In the event of conflict, these Terms govern service participation, while the Privacy Policy governs data handling.

Updates to Terms

We reserve the right to update these Terms and Conditions at any time. Continued use of the website and services constitutes acceptance of any changes.
